Do this by selecting a location from the Location Tag drop-down menu.<\/li>\n\n\n\n<li>Select the \u201cToilet Feedback\u201d on the Form Type drop-down menu. You can choose between landscape or portrait orientation.<\/li>\n\n\n\n<li>Select the type of fault from the Fault Type drop-down menu.<\/li>\n\n\n\n<li>You also have the option to select a background that will be used for the Toilet Feedback Form. You can choose white, default background, or upload a custom background.&nbsp;<\/li>\n\n\n\n<li>Once all the information has been entered, click on the \u201cSubmit\u201d button on the bottom scanned using any scanner to access the form.&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Once scanned, requestors can provide their rating.&nbsp;<\/li>\n\n\n\n<li>If a requestor rates the toilet \u2018poor\u2019 or \u2018very poor,\u2019 they can state the reason as to why they gave the said rating by selecting from issue given.&nbsp;<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-image
